Best for: catching small problems early.
Naples · Bonita SpringsElan is private, RN-led care management for older adults and the families who love them, and for adults recovering at home after surgery. One nurse who knows your history, keeps everyone on the same page, and is the person you call.
Most families are juggling specialists, new prescriptions and a parent who says "I'm fine." Others are facing surgery and a stack of discharge instructions. Adult children often live out of state. Elan fills that gap with one steady, licensed professional.
Think of Elan as an extension of your family, with an RN license and the time to help.
For older adults and their families. Three membership levels, each with a 60-minute RN visit in the home every week. Pricing is shared during your free consultation.

The first hours after a procedure can feel overwhelming, especially when you're recovering without nearby family or simply want the reassurance of experienced nursing support.
Elan Care Management offers personalized, extended-hour in-home postoperative nursing services in Naples and Bonita Springs. Whether you're recovering from an elective procedure, orthopedic surgery, or another qualifying outpatient operation, you'll have a dedicated registered nurse by your side to help you navigate the early recovery period.
Focused nursing assistance during the initial hours at home, providing professional observation, guidance, and reassurance as you settle into recovery.
Extended one-on-one nursing support for patients who would benefit from additional observation, personalized assistance, and guidance during the early recovery period.
A longer period of dedicated nursing support for patients whose recovery plans call for more sustained assistance, observation, and coordination with their healthcare team.
Every postoperative care plan begins with a consultation to determine whether Elan's services are appropriate for your procedure, medical needs, and surgeon's discharge requirements. A planning visit in your home 1 to 2 weeks before surgery is also available.
In-home nursing services begin upon arrival home following surgery. Transportation from the surgical facility is not included. Any required responsible adult or caregiver arrangements must be made in accordance with the surgical facility's discharge instructions. Elan's services do not replace emergency care or medically necessary hospital-level monitoring.

Founder | Elan Care Management
Experienced nursing. Personal attention. Someone in your corner.
I've spent my nursing career helping patients and families navigate some of life's most challenging healthcare moments. Along the way, I've learned that good healthcare isn't just about medical appointments, medications, and treatment plans. It's about having someone who listens, understands the bigger picture, and makes sure nothing important gets overlooked.
As a registered nurse with a background in complex care management, chronic disease management, and patient advocacy, I've worked closely with patients, families, physicians, and healthcare teams to coordinate care and help people feel more confident managing their health.
But I've also seen how overwhelming healthcare can become, especially when families are trying to coordinate care for an aging parent or someone is returning home after surgery.
That's why I created Elan Care Management.
I wanted to offer something more personal. A nursing relationship built on trust, consistency, and genuine connection. One where clients aren't just another appointment on a schedule, and families know they have an experienced professional they can turn to.
Whether I'm helping an older adult remain independent at home or providing dedicated nursing support following surgery, my approach is the same: thoughtful, individualized care with attention to the details that matter.
At Elan, my goal is simple. To make navigating healthcare feel a little less overwhelming and a lot more personal.
Because everyone deserves to have a nurse in their corner.

Throughout my nursing career, patients and families have consistently valued having someone who takes the time to listen, answer questions, and make sure they understand what is happening with their care.
Patients often tell me how reassuring it is to have a nurse they can trust—someone who can help them make sense of medical information, communicate concerns, and navigate what can otherwise feel like an overwhelming healthcare system.
The feedback that has meant the most to me throughout my career is when patients tell me they feel genuinely cared for—not rushed, not treated like a number, and not forgotten once the appointment is over.
Choose the level of support that fits your recovery: Essential Recovery (4 hours), Enhanced Recovery (6 hours) or Premier Recovery (8 hours). Nursing care begins when you arrive home from surgery, and a planning visit in your home 1 to 2 weeks before surgery is also available. Every plan starts with a complimentary consultation to confirm Elan is right for your procedure and your surgeon's discharge requirements.
Postoperative care is booked by the visit, with no membership required. Transportation from the surgical facility is not included, and any responsible adult or caregiver the facility requires must be arranged separately.
Memberships begin with a three-month commitment, then continue month to month. Every level includes a 60-minute weekly RN visit at home. Foundation Oversight adds RN phone access, with calls returned the same business day by 6pm, and a quarterly family update. Signature adds escort to select appointments, medication oversight across all prescribers, priority same-day response during business hours, and a monthly family update. Elite Advocacy adds up to four escorted appointments a month, hospital and ER liaison during business hours, seasonal snowbird transition management, and quarterly family conferences in person or on Zoom.
